TORONTO – As Ontario moves toward the ambitious Primary Care Action Team (PCAT) goal of connecting every resident to a primary care provider, Ontario Health Teams (OHTs) and regional leads are facing a significant infrastructure challenge. While the mandate provides the vision for universal attachment, the practical execution requires a digital ecosystem that can scale without increasing the administrative burden on an already overextended workforce.

On Tuesday, March 31st at 1:00 PM EST, industry leaders will convene for an executive-level briefing on deploying modular, HALO-aligned digital capabilities to meet these provincial requirements. The session will move beyond high-level policy to explore the specific technical levers – from app catalogue discovery to AI-powered navigation – that allow OHTs to operationalize primary care access.

The panel features Jijesh Devan, VP product marketing, IDENTOS (pictured left), Dr. Ryan Doherty, PhD, president & founder, Empower Health (pictured center), and Ted Alexander, VP, Amplify Care (pictured right). Together, they will discuss how to bridge the gap between unattached patients and available care through interoperable, secure, and provider-friendly digital tools.

Key discussion pillars include:

  • Reducing Digital Burden: Implementing single sign-on (SSO) and curated app catalogues to return time to clinicians.
  • AI-Powered Patient Navigation: Leveraging intelligent routing to ensure patients are directed to the right care at the right time.
  • Data Infrastructure: Actively managed directories of primary care clinics & clinicians to support regional care coordination & primary care attachment
  • Secure Caregiver Delegation: Empowering the broader care team and family caregivers through secure, HALO-compliant access.

This session is designed specifically for OHT executive leads, PCN clinical leads, and digital transformation teams responsible for regional primary care enablement. Attendees will gain a clearer understanding of how to build a unified digital front door that meets Ontario’s rigorous security standards while improving care coordination for all.
